A young man had introduced a dildo into the anus before going to bed and was unable to remove it. He attended our emergency department for retrieval the next day. He did not divulge his sexual orientation or history. Physical examination revealed tachycardia, but no rectal bleeding, abdominal tenderness, or muscle guarding. Laboratory study revealed leukocytosis. Plain KUB radiograph revealed increased density in the pelvis representing a foreign body with two metallic inner cores in the sigmoid colon (Figure 1). The glans penis shape confirmed that it was a dildo. Pelvic Computed Tomography (CT) showed no regional free air or abscess (Figure 2). A 3-dimensional reconstruction of the image to guide the surgeon for surgical planning was done. However, the dildo was spontaneously passed from the anus while the patient was waiting for management. Follow-up KUB did not show the density and the patient was discharged without further event.
